For Support Coordinators

A Straightforward Referral Partner

What we deliver, what we’re registered for, what we don’t do — and a direct number for each team. No chasing required.

Answered 24 hours a day, 7 days a week — by a real person, not a machine.

Ask for whoever you need — nursing, SIL, programs, aged care, or the coordinator looking after a particular participant — and our team will put you through. If you’re ringing about an urgent placement out of hours, ring anyway.

To message a specific coordinator directly, use the coordinator form. Referrals go through the referral form, or just call.

What we do

Six service lines, one team. If you want to know whether we can take a particular participant, the fastest answer is a phone call — but this is the shape of it.

Alongside those: group programs 7 days a week, and Support at Home aged care, with self-management through Trilogy Care.

Incidents and complaints are tracked in DCIQ

If you refer to us, this is the part you’ll care about. Since July 2024 every incident, complaint and compliment has been logged, investigated and closed out in DCIQ — the risk and incident management platform built by RLDatix and used by hospitals and health services internationally.

It means reportable incidents are captured properly, patterns surface early rather than at audit, and if you ask us what happened with something you raised, we can tell you. RLDatix’s announcement.

What we’re registered for

We have been a registered NDIS provider since 2021. A list of services is easy to write, so here is the version you can check. These are our approved registration groups — check them yourself on the NDIS Commission register against ABN 78 943 536 090. Approved through to 8 September 2028.

Supplementary modules we’re audited against

Some supports need more than a registration group — they need a supplementary Practice Standard module, assessed at audit. These don’t show in the groups list, so they’re easy to miss when checking a provider.

Module 2A — Implementing behaviour support plans. We hold this, so we can take a participant with a behaviour support plan, including one containing regulated restrictive practices. It’s a breach of the NDIS Rules for an unregistered provider to use one.

Module 5A — Assistance with SIL. Since 1 July 2026 SIL providers have had to be registered and meet the SIL Practice Standards, built around participant voice and Supported Decision Making.

What we are not registered for

Probably more useful than the list above, because it tells you when to send someone elsewhere:

  • Plan Management — not offered.
  • SDA — we support people living in SDA, but we’re not the housing provider.
  • Support Coordination — registered, deliberately not delivered.

We also don’t write behaviour support plans — that’s a specialist practitioner’s role. Bring us the plan and we’ll implement it.

Working with us

Three things worth knowing

RN

You’re not carrying the clinical risk

A registered nurse assesses every participant before service starts, writes the care plan, determines the team and trains the workers to that individual plan. Nurses re-assess on schedule and on change, are reachable on any shift including after hours, and liaise with GP and hospital with consent.

Assessments and reports come back current, and you aren’t the one judging whether a worker is competent for a complex participant. The full process is here.

We stay in our lane

You coordinate the plan. We deliver the supports in it and report back to you. We don’t case-manage around you, and we don’t take conversations that belong with you.

‘Implementing provider’ is a narrow Commission term for following a behaviour support plan. It has nothing to do with implementing an NDIS plan. That’s yours.

!

No conflict of interest, by design

We hold Support Coordination registration and choose not to deliver it, and we don’t offer Plan Management at all. We’re never coordinating a plan we also deliver against, so we’re not a conflict for your recommendations.

If something’s off, tell us early — you’ll often hear about a problem before we do, and we’ll come back to you on what we’ve done about it.
