Getting started

How a referral works

1. Get in touch. Anyone can refer — participants, families, support coordinators or other providers. Use the referral form or just call us.

2. We talk about support, not funding. We’ll ask what a good day looks like, what’s hard right now, and what you want to be different. No obligation, and no pressure.

3. We check we’re the right fit. If we’re not the right provider for you, we’ll say so and point you elsewhere. That happens, and it’s better said early.

4. A nurse meets your family. Before support starts, one of our nursing team meets every family we work with. Nothing about your situation should be a surprise on day one.

Questions about your funding? Ask these people, not us.

We’re a service provider. We get paid out of your plan — which is exactly why we shouldn’t be the ones telling you what your plan should cover.

Your support coordinator is the right person for what your plan includes and how to use it. Your plan manager handles budgets and invoices. The NDIA (1800 800 110) can confirm anything about the plan itself.

If you don’t have a support coordinator and think you need one, tell us and we’ll help you find an independent one. We don’t deliver Support Coordination ourselves, so we’ve got nothing to gain from who you pick.

What do you need from me to get started?

Enough to know whether we can support you well: what supports you're after, and confirmation from you or your coordinator that they're funded. We don't need to see your whole plan.

Can I switch to Lionheart from another provider?

Yes, and people do. We'll work with you and your coordinator on the handover, including any care plans or clinical information, with your consent. Several of the people we support came to us from somewhere else.

Do you offer Support Coordination or Plan Management?

No — by choice, and there’s more on why under our registration list.

Do you work with self-managed and plan-managed participants?

Yes — self-managed, plan-managed or NDIA-managed, all three.

We've been told our family member is too complex. Should we still call?

Yes. Please call. That's the exact situation this organisation was built for.

What we’re actually registered for

Anyone can say they're a registered NDIS provider. You can check ours on the NDIS Commission's public register — search our ABN, 78 943 536 090. Our registration is approved through to 8 September 2028.

A note on that last one

We hold Support Coordination registration, and we choose not to deliver it. We don’t offer Plan Management at all. That means we are never coordinating a plan that we also deliver against — there is always someone independent of Lionheart checking that a recommendation is in your interest.

It costs us work. We think it’s the right way round.

Alongside those groups we are also assessed against supplementary NDIS Practice Standard modules, including Module 2A: Implementing behaviour support plans and Module 5A: Assistance with Supported Independent Living. Supplementary modules don’t show as registration groups — they’re additional standards a provider has to be audited against to deliver those supports at all.

Registration means independent audit, worker screening, incident management and compliance with the NDIS Practice Standards. It’s a fair thing to ask any provider to evidence — including us.
