Care starts with knowing you
Every person has their own story, their own goals and their own way of living. We take the time to get to know you before we begin supporting you — what a good day looks like, what you still want to be doing yourself, and where a hand would actually help.
Receiving support doesn’t mean giving up your independence. Whether you need a little help or more comprehensive support, the aim is the same: that you live safely and independently, in the place you call home, making your own decisions about your own life.
We’re locally owned and family-operated. The person who answers the phone is in Gladstone, and so is the person who comes to your door.

Aged care support begins with an assessment through My Aged Care. If you haven’t done that yet, start at myagedcare.gov.au or ring 1800 200 422.
If the process feels like a lot, ring Lynette first. She’ll talk you through what to expect before you speak to anybody official.
A wellness and reablement approach
Wellness and reablement means starting from what you can do rather than what you can’t, and supporting you to keep doing it — or to get back to it after a setback. It shapes how we assess, how we plan and how our support workers are asked to work.
We start from your strengths
Your first assessment records what you manage on your own, what has become harder, and what you want to be able to keep doing. That’s the baseline everything else is measured against.
We support, we don’t take over
Where you can do part of a task, our workers assist with the part you can’t — steadying you in the shower rather than showering you, or carrying the basket while you hang the washing.
Goals are written down
Your care plan names what you’re working towards in your own words, not just the tasks. Getting back to the shops on your own is a goal. So is managing your own medication again.
Plans change as you do
We review with you at regular intervals and again whenever something shifts — a fall, a hospital stay, a new diagnosis, or a good stretch where you need less than you did.
Short-term help to get back on your feet
After a hospital admission or a fall, more support for a while can be what prevents a permanent step down. We plan that as a period with an end point and a goal, not a new normal.
We bring in the right people
Reablement often needs an allied health professional — physiotherapy, occupational therapy, a home safety assessment. With your consent we coordinate with them and with your GP so everyone works from the same plan.
Less support is a good outcome
If a review finds you need fewer hours than you did, we’ll say so and reduce them. That is what reablement working looks like, and we’d rather tell you than keep billing for support you’ve outgrown.

Personal care and daily living
Assistance with showering, dressing, grooming, mobility, medication prompts and everyday routines — delivered in a way that protects your dignity and does with you rather than for you.
Domestic assistance and meals
Keeping your home clean, comfortable and safe: household tasks, laundry, shopping, and preparing meals that suit your tastes and your diet.
Social support and community connection
Life is more than receiving care. Outings, appointments, hobbies, social activities, or a conversation over a cup of tea — staying connected to family, friends and Gladstone.
Clinical and nursing support
Where clinical care is needed we coordinate it through our own nursing team — medication management, wound care, health monitoring and other specialised support, so your care stays joined up rather than split between strangers.
Support at Home, and self-management
Support at Home is the Australian Government programme for aged care delivered in your own home. It replaced the Home Care Packages Program, so if you were told you have a “home care package”, this is the same thing under its current name.
If you self-manage (or want to), we work alongside Trilogy Care on the administration, so more of your budget goes toward actual support rather than paperwork.
Trilogy handles the compliance and the paperwork; we deliver the support. Two organisations, two jobs — which also means your administration isn’t being done by the people billing against it.

Do I need an assessment before you can help?
Yes — aged care support starts with an assessment arranged through My Aged Care. Start at myagedcare.gov.au or ring 1800 200 422. Ring Lynette on (07) 2140 9779 first if you'd like someone to explain the process before you begin it.
I have a Home Care Package — is that still a thing?
Your support continues. Support at Home replaced the Home Care Packages Program, and existing arrangements were carried across. If you're unsure what applies to you, ring Lynette and she'll check rather than guess.
What is self-management, and is it right for me?
Self-managing gives you more say over who supports you and how your budget is spent, with Trilogy Care handling the compliance behind it. It suits some people and not others, and it isn't a decision you have to make on the phone. Lynette will talk you through both.
What does “wellness and reablement” actually mean for me?
That we look for what you can keep doing yourself and support that, rather than taking tasks off you. In practice it means your care plan has goals in it, we review it as things change, and if you need less help than before we'll reduce it.
Can Lionheart provide nursing support as well?
Yes. We have our own nursing team in Gladstone, so clinical support and everyday support come from the same organisation and the same care plan.
